Unreasonable Work Pace: Setting a work pace that is too high or creating a stressful work environment can lead to workers rushing which increases the chances of an accident. Lack of Safety Devices: Removing safety devices or having safety devices that are not properly functioning can be a recipe for disaster. For example if the forklifts rollover protection is not engaged or not functioning correctly the operator can be thrown from the machine or pinned underneath if it should over turn.
